Reports directly to: Social Works Supervisor
Staff Reporting to Position: None
Department: Social Work

Job Role:
To provide social work services, psychosocial support, and counseling for the parents/caretakers and patients in key areas of child/family development and well-being, emotional and spiritual health, and financial sustainability

Key Result Area Activities
• Assessments: Conduct the patient and family assessments for each patient including all aspects of socio-economic background that will be important to providing quality services.
• Conduct Socio-Economic Assessments of each patient’s family.

• Develop a comprehensive understanding of the social service needs of the patient’s family.
o Provide feedback to the department supervisor on the family’s socio-economic situation for use in determining payment plans for private and adult patients.
• Continually identify, enroll and assign patients to sponsor organizations including CCHU as a primary sponsor.
• Together with the department supervisor and finance department, process and follow-up on all outstanding invoices from sponsor organizations, private and adult patients. on monthly basis.
• Actively manage a caseload of assigned patients/families in the provision of Social Work services offered at the hospital and links to the community.
• Provide orientation to all patients on the operations of the hospital and services provided including spiritual ministry, patient sponsorship, patients’ rights, and responsibilities, in liaison with the various departments.
• Provide counseling to patients and families on a wide range of life and health issues (such as child development and wellness, financial sustainability, HIV/Aids counseling, etc.
• Actively manage and track a caseload of assigned parents/families.
• Complete regular patient home visits and meetings with parent support groups.
• Follow-up with patients on issues presented through the Assessment process.
• Follow-up patients who miss medical appointments. Endeavor to do at least one patient follow-up per week.
• Work with your supervisor and finance department to help ensure private patients pay their bills as per the agreed terms.
• Together with the CURE storyteller, write at least 1 patient story (especially success stories) with photos per month as a resource to draw from when requested.

• 3. Community Linking
o Obtain patients’ consent to share their information with sponsor organizations, CBR and, other support organizations.
• Link patients and their families to CBR and other support organizations in their respective communities or regions.
• Active involvement with parent support groups in the assigned region.
• Build relationships with key social services organizations, CBR groups, and other support organizations available in the respective regions to link our patients.
• Advocacy for vulnerable children/awareness in the community around children with neurological health conditions (HC, SB, EC, etc.)

• 4. Ensure smooth running of mobile clinics
• Work with the IT and Health Information office to obtain a patients list for each clinic to be able to establish the expected number of patients, find their files.
• Coordinate with respective departments to establish the number of staff needed for the clinic.
• Share the annual clinic calendar with the host organizations or hospitals and confirm their readiness to host the clinic.
o Registering Patients as per established protocols Handle all administrative and housekeeping issues (including transport, accommodation, meals, and other payments for support hired teams at the clinic
• Prepare Satellite Clinic report and accountability
• Share with supervisor and other departments matters that need improvement or other forms of action for the next clinic(s).

o Submit routine reports documenting various activities
• Work as a contact person of the hospital and the patient sponsor organization where necessary.
• Together with the finance department, prepare reports for patient’ sponsor organizations as per the agreed guidelines.

• Spiritual Ministry
• Participates in the hospital’s Spiritual Ministry programs.

o Performs any other related duties as may be assigned by Supervisor

Essential Qualifications
o Completed Degree in Social Work and Social Administration, Social Sciences, or any other related field.
• At least 3 years experience working as a social worker most preferably in a health care setting
• Certificate in Guidance and Counselling an added advantage.
• Computer knowledge in Ms. Office, e-mail, and internet.

• Good teamwork, negotiation, and people management skills.
• Good oral and written communication skills
• High-level planning and organizational skills including efficient and effective time management and the ability to prioritize tasks.

• Ability to travel long distances for satellite clinics and patients’ home visits/follow-up.

• Experience in Social Work/Community Based Rehabilitation programs is an added advantage
